Job description
The RN/LPN Administrator is responsible for assessing and reassessing the nursing needs, including physical, psychosocial, and environmental needs of clients and their families. The RN develops an appropriate and individual care plan, collaborating with the client's primary physician as needed and is responsible for teaching/counseling clients regarding self-care and evaluating the client's response to care provided. Acting as clinical care coordinator for assigned individuals/families, the RN initiates referrals as necessary, supervises and directs Home Companions, Home Health Aides, Certified Nurse Aides, families and other caregivers regarding care of the client. The position is part time with a flexible schedule.

RN/LPN Responsibilities:

Medication Management
- Prepare medication sets.
- Administer physician-prescribed medications.
- Offer medication reminders.

Patient Assessment
- Evaluate and document the patient’s condition during each visit.
- Review medications and vital signs as part of the assessment.

Patient Education
- Educate patients and their families on effective home health care practices.
- Suggest tools or devices that can enhance the patient’s quality of life.

Transportation
- Maintain reliable transportation for visits.
Requirements:
- Current Kansas State License
- Prior Home Health experience (Isn't required but helpful)
- Must have reliable transportation and a valid state driver's license
- Adhere to HIPAA and maintain client confidentiality
